As a paneled provider, ProCare agrees to treat the injured employee within 24-48 hours of the
referral from a paneled physician. ProCare agrees to provide timely, objective reports within three days of treating the injured employee and will provide weekly reports to all rehabilitation team members to maintain the channels of communication. ProCare follows a time-limited outcome driven approach to rehabilitation. The injured worker receives only those therapies appropriate, for the period of time services are appropriate, and treatments follow objective goals relating to level of function and job requirements.

Evaluation and therapies provided include:

Outpatient Rehabilitation Services: A wide range of services are provided by licensed physical and occupational therapists to clients in acute through chronic phases of injury and/or illness recovery. A focus on return to function is the basis for all services provided and is followed from the initial treatment session.

Functional Capacity Evaluation (FCE): An objective measure of the injured worker's current physical impairment, level of symptom magnification, and functional abilities. Information provided by the FCE includes the injured worker's current physical demand category, ability to return to work in the previous job position or modified duty, any reasonable accommodations and the pot~ntial for improving the person's physical abilities, and potential for return to work through a Work Conditioning or Work Hardening Program. Results of the FCE can be used by the rehabilitation team to make safe return to work decisions. The process of completing an FCE is approximately four hours of one-on-one therapist time.

Work Conditioning Program (WCP): A program designed for the worker who continues to remain out of work or on light duty past the acute injury phase and initial therapy treatment (typically less than three months). The program emphasizes strength, flexibility, endurance, proper body mechanics, lifting techniques, postural corrections, education for prevention of re¬injury, and work simulation. The program progresses in intensity and is specifically designed to meet the needs of the worker and job requirements.

Work Hardening Program (WHP): A program designed for the worker who continues to remain out of work past the expected length of normal treatment (typically greater than three months) and demonstrates pain behaviors and symptoms magnification. The program emphasizes strength, flexibility, endurance, proper body mechanics, lifting techniques, postural corrections, education of prevention of re-injury and pain management, and work simulation. The program includes an educational component that focuses on the behaviors and barriers to return to work. The program is structured to simulate a work schedule and the worker typically progresses from four hours to participation up to eight hours per day.

On-site Rehabilitation: The range of therapy services provided by ProCare Rehabilitation can be provided at the work site. The services provided will be dependent on the injured worker's
needs and the employer's accommodations in which to provide the services. The services will be determined on a case by case basis in conjunction with the rehabilitation team. WE ARE ALWAYS AVAILABLE!

We can provide Technical Resources
Consultation: An analysis of an industry to determine the types of potential causes of injures or guidance in the process of developing ergonomic or injury prevention programs. This can include a review of OSHA 200 logs and a walk-through of the plan to identity areas of concern.

On-Site Job Task Analysis (JTA's): Analysis of the work site to define the physical/functional demands of the job. The information can be utilized to develop functional job descriptions, provide the rehabilitation team objective information about demands of the job in which safe return to work decisions can be made, to develop modified duty positions, and in developing post¬offer employment screening.

Work - Rest Cycling Analysis: Analysis of the integral cycle (s) of a job with a goal of preventing and/or reducing injuries or defending against claims of cumulative trauma disorders (CTD's). The analysis defines activity times versus interrupted activity/posture, intensity of loads, frequency of activities, etc. The final analysis is compared with existing national research of repetitive motion injuries and if a problem exists, recommendations can be made for improving the job design or work station.

Ergonomic Assessments: An overall analysis of the job from an ergonomic and biomechanical perspective. The analysis will include all aspects of the job to identify risk factors or ergonomics stressors. The analysis can be used to re-design the job to decrease injury rates.

Legal Testimony: ProCare provides legal support including documentation, for cases relevant to physical therapy, occupational therapy, and functional assessments relevant to workplace analyses and impaired individuals. Applications include workers' compensation, disability, ADA, and similar regulatory systems.

Back or Cumulative Trauma Education Programs: Management and employee training designed to provide pro-active injury prevention strategies including posture, lifting techniques, stretching, and exercise.

Tool Box Talks: Discussions with the employees at particular work stations to provide brief informal, reminders about working smart and working safe. These often are a few "hints" or reminders about proper body mechanics.

Post-Offer Functional Screening: A functional screening of an employee after an offer of employment has been made. The screening establishes a "baseline" of data concerning an employee's physical capabilities. The process is utilized to assure that the employees demonstrate the physical abilities to meet the essential physical demands of the job upon hiring or on an annual basis to reduce the incidence of on the job injuries due to mismatch between the workers strength and the job requirements.
